How to Get a Library Card

Persons residing, owning property, working, or attending school in Ohio are eligible, with proper identification, to register for a Mary L. Cook Public Library card.

 

Adults, patrons age 18 and over, must present valid photo identification with the current, correct, mailing address. If the photo ID does not have the current mailing address, the patron may present a utility bill, bank mailing, or other business mail as proof of address.

 

A patron under age 18 may register for a library card, but his or her parent or guardian must be present to sign the registration form, and the parent or guardian must present the required identification and accept responsibility for any materials borrowed on the child’s card. The parent or guardian must present the required identification as outlined above to satisfy the requirements for identification for the patron under age 18.

 

Patrons under age 12 will not receive a physical card, but will instead verify his or her name. Responsibility for the materials a child chooses to borrow rests with the parent. A parent or guardian, in the presence of a library staff member, may give permission for his/her child under 18 years of age to access the Internet and/or borrow juvenile DVDs by signing the child’s registration card. A parent or guardian, in the presence of a library staff member, may give additional permissions for his/her child between 12 and 18 years of age by completing the “Parental Permissions for Minors Age 12 and Above” form.

 

REGISTER FOR AN eCARD

If you do not currently have a Mary L. Cook PL card but would like to have access to digital material that is available on OverDrive (we are in the Ohio Digital Library consortium) without physically visiting the library to register for a regular library card, we offer an eCard that you can register for online. When you register, you will receive a barcode that begins with PACREG. This PACREG number and your PIN will allow you to access your Mary L. Cook account and to set up an OverDrive app (Libby App for iOS and Android users; OverDrive app for computers or KindleFires). OverDrive offers ebooks, e-audiobooks, magazines and some older films. It would be very helpful if you include your email address on your registration. In compliance with the Children's Online Privacy Protection act, you must be 13 or older in order to complete this registration. As with a regular Mary L. Cook Public Library card, you must be a person residing, owning property, working, or attending school in Ohio to be elibible for an eCard.
